Self-employed and retired Medical Association of Georgia members now have access to a lower-cost alternative to private and ACA exchange plans. When you establish a self-funded MAG One Health Benefits plan, you and your dependents can get great care and may save money.
Eligibility and Requirements for the UnitedHealthcare® Group Medicare Advantage Plan
Important information: Medicare requires the following for you to be enrolled in the Group Medicare Advantage (PPO) plan insured by UnitedHealthcare®.
- You must be entitled to Medicare Part A and enrolled in Medicare Part B
- You must continue paying your Medicare Part B premium
- You must have a permanent street address (this cannot be a P.O. Box)
- You must have your Medicare ID number
- You must live within the 50 United States, the District of Columbia, or U.S. territories
If you are not enrolled in Medicare Parts A and B, and/or you live outside the 50 United States, the District of Columbia, or U.S. territories, you should contact Social Security at 1-800-772-1213, TTY 1-800-325-0778, between 8 a.m.–7 p.m., Monday–Friday, or call your local office.
Eligibility and Requirements for the MAG One Health Benefits Plan
- Must be self-employed or a business owner with no W-2 employees.
- Must have a federal tax ID.
- Must be a member of the Medical Association of Georgia.
- Applicants required to answer a short major medical issues screening questionnaire.
